The idea of Pay for Performance was developed by various groups in 2001.

They included the government initiated agencies, health insurance organizations, physicians and other private companies. The system offered health payments for improved health care for those with chronic illness such as diabetes. The system targeted rewarding the improvement in the management of patients. Later on, the health approach was adopted in the rest of the world. Most of the healthcare services providers have started adopting the Pay for Performance scheme. However, the pursuit to adopt Pay for performance system has raised credible concerns among the health care providers (Peter, 2002). Among them is the concern on the real underlying objective of the approach. Although, the main goal of the approach is improving the quality of the health services, physicians and hospitals fear that cutting down of the cost of care is the main underlying goal.

According to many, the approach is oriented to only achieve the safety goals of the patients in expense of their improved profitability objectives. A pilot study cases show that, Pay for Performance program does not save on cost of care. This is because of the added cost on the administrative management. Implementing the system calls for review of health records and substantial investment in electronic health data managing devices. Therefore, implementation of the system proves to be expensive, especially to the small scale organizations (Lance, 2007).

The stakeholders in the provision of the health care have also raised a concern on the validity of evaluating the data for quality improvements.
