Storm and sewerage to connect into existing manholes Site Details Contractor: Project name and site address: Site manager: Transfer of Information from Client/Contractor to Ground Works Sub-Contractor Investigations will be undertaken at the site to determine the engineering features of the ground on which the foundation of the structure will be laid and offer details on bearing capacity. This information shall be availed to the groundwork’s sub-contractor. In case the site investigation establishes any contamination of the ground due to prior industrial uses, this information will be availed to the groundworks sub-contractor. Drawings of utility services will be provided indicating the location of any buried services. If work has to be done near or under overhead lines, the line operator will be consulted to establish whether the line can be diverted or made dead. In case the lines are left live, the plant is chosen or modified to prevent it from reaching the lines. Attendances Before the commencement of any work on site the client/contractor shall:- Use necessary cable detection instruments to precisely determine and mark service locations on the ground. Ensure that transport routes are watchfully planned so that plant does not have to draw near the boundary of the excavation. These routes should be clearly demarcated using fences or baulks of timber. Ensure the site is accessible from the public road onto and near the site before vehicles enter site, considering the site traffic management framework, the selected work area to be used by delivery vehicles and the storage area for construction materials. This information will be clearly identified and communicated to the ground works sub-contractor. Avail welfare facilities to the groundwork sub-contractor. Work Activity Pre-start checks: The pre-start checks will be done to ensure:- Availability of equipment and plant to be used; welfare facilities; ground conditions and proximity hazards. Check the excavation area for underground and overhead services before any work begins. Details of the contract: Site excavation and casting of strip foundations. Construction of and linking them to existing manholes (information available in working drawings). General Method Statement (COMPANY NAME) undertake excavation works for (DEFINE CLIENT). The work involves site excavation and casting of strip foundations. The works are undertaken at (LOCATION). The work done and supervised qualified, skilled, competent and professional personnel at the work site depending on the task/job risk assessment. It is the expectation of (COMPANY NAME) that all clients shall act in accordance with the statutory requirements of Health and Safety Regulation and notify every one of all risks and hazards which might affect their health and safety whilst at the location of the client. The excavation works are potentially risky by nature. The training in production work procedures, secure systems and management systems guarantee that works are finished safely. Management Structure All operations are under the management of (INSERT NAME) located at (INSERT ADDRESS). All site operatives are under the direct supervision of the foreman on site. All operatives are trained and experienced in their respective duties. Work commences at the site only when a foreman and no less than one other operative is present. The work instructions details the specific number of operatives required for each task. Communications Regular communication between site operations and the client is crucial all the time. Every foreman is equipped with a mobile telephone to ensure communication does not break down. Operational instructions are communicated through electronic, written or verbal means. Personnel that need access to sewers must complete restricted space training. Other site operations: in case co-operation and co-ordination with other site operations is necessary, it must be specified. Date/s and time of works The works will be undertaken on weekdays between 7.30 a.m. and 6.00 p.m. depending on weather and existing daylight permitting. Changes in work patterns will be discussed with client, site owners or others, where applicable. The nature of the work and the locations dictates that health and safety measures for safe access are evaluated at the work site in consultation with the client. Duration: One (1) week/Seven (7) days Emergencies: Appropriate personnel with training in emergency procedures will be present at the site. First Aid: All vehicles used for the works will be equipped with a squad first aid kit. Any operatives working more than 30m from a vehicle will carry a personal first aid kit. In case the team is working over 30m from the vehicle, the first aid kit will be located at… Vehicles/machinery: 360 deg excavator Dumper The team leader controls all vehicles and conducts regular inspections both during site and supervisory visits. Regular maintenance is done on the vehicles to ensure they meet the requirements of road traffic rules. Vehicles are only operated by operatives holding a relevant and current driver’s license. All machinery is inspected from time to time by the supervisor during supervisory and quality and supervisory visits. Faulty equipment is removed from service and repaired before being returned to field operational use. The operations procedures include all plant and machinery operating instructions. These procedures are regularly checked and reviewed to include current knowledge and experience as appropriate. Clients’ personal health and safety procedures and standards can take preference when of higher standard. Method: Excavations on this site will be made to a depth of 1.5m. As identified, the ground conditions will permit the trench sides to stand perpendicularly without using shutters. Strip foundations shall be excavated and concrete foundations placed on the same day. No operatives shall be allowed to enter excavation. The concrete will be poured straight into the excavation and spread using rakes. Lime will be used to clearly mark the area to be excavated on the surface. Where appropriate, physical barriers will be raised to cover the excavation. The ground works sub-contractor will communicate with the client/contractor to make sure that any live services on the site have been marked and clearly demarcated. Where appropriate, required hand-digging measures will be employed to reveal existing services. Using a 360 deg excavator, a 600mm wide by approximately 1.5m deep excavation will be dug. The debris dug from the excavation will be stacked into a dumper and removed from the working area to prevent surcharge on the side of the excavation. The dumper will be fitted with roll-over safety and the dumper driver must be strapped with a seat belt. No edge tipping is to occur. The services of a banksman will be required to help in the secure operation of the plant and to monitor the area being excavated to make certain that no undiscovered services exist. Before placing concrete using the excavator, any loose soil from both the base and the sides of the trench will be removed. The formation level will be studied and passed. The excavation will be filled with ready mixed concrete to the necessary level as outlined using a laser. Where it is impossible to pour concrete directly into the excavation using chutes, the concrete will be put in a clean excavator bucket and placed in the strip foundations. The side of the excavation will be supported using a drag box together with a 360 deg 15 tonne excavator. In case ingress of water is encountered at any time when the excavation is open, a pump will be used to remove the water. Work will begin at the deep end. Using a 360 deg excavator, the trench will be excavated to the recommended width (1.2m) and the drag box will be placed into the pre-dug trench. Excavated debris will be removed and taken away from the site using a dumper. Edge protection will be connected to the drag box or left to remain free standing. Personnel must ensure that no unsupported section of the trench is used in gaining access. A secured ladder extending 1m above the step off level will be used to access the excavation. Granular material will be placed under the sewer pipes to a minimum depth of 150mm. The trench will be excavated by digging in front of the drag box and the excavator pulling the box forward using the leading strut. Workers movements must be confined to the protected area i.e. within the drag box to place the sewers. Prior to entering the foul manhole to connect the pipe, workers must utilize a gas tester to verify the atmosphere and check for any risks of toxic gas, oxygen deficiency, explosion or fire. In case of flammable or toxic gases, such should be purged from the hole and the atmosphere should be checked to confirm that purging is effective. When operatives are all clear, the drag box will be carefully taken out by lifting and dragging the front strut. The material surrounding the pipe will be put immediately after laying, jointing and inspecting the pipe as soon as the pipes have been laid, jointed and inspected. Specific backfill from the trench excavation without large stones and vegetative matter will be used to finish the backfilling of the trench. Mechanical compaction will not be done until the fill is approximately 450mm over the pipe-work. The groundworks foreman will be responsible for ensuring the excavation is safely conducted: at the beginning of each shift; after any event that may have affected the stability or strength of the excavation; and after any debris accidentally falls or is dislodged. In case the excavation work continues beyond seven days, the foreman is to complete an inspection record. Plant details: The foreman will examine the through assessment certificate for the excavator and the check up report on the dumper. Site risk assessment: The team leader will fill-in an on site risk assessment to note the weather and any change of events in the course of the operations, notify the workers and review day-to-day events for significant site changes. The team leader shall ensure the health and safety all persons accessing the site or those that might be affected by the excavation works. Personal Protective Equipment: All personnel must wear the following: Safety wellington boots during concrete works, gloves, a high visibility vest and a safety helmet. Site Rules: All personnel will be notified of and are supposed to conform to the contractor’s site regulations. Specific Site Hazards: Before the commencement of any excavation works, specific site hazards will be determined and tackled. Access to the Work Area: The contractor shall ensure that a safe means of access to the site is available. Welfare facilities: The contractor/client must ensure that welfare facilities are available. Falls into excavation: The groundworks sub-contractor must avail and maintain fencing to avert operatives from falling into the excavation where necessary. Risk prevention Equipment will be properly maintained and kept safe working order. The amount of fuel on site at any given time will be kept minimum: fuel sufficient for a day’s work will be transported to the site; fuel will be kept away from buildings when storing or refueling.
