Based on the setting, I expect that more than half of the populace who have the condition has not controlled it. This is a risk as they are prone to issues such as stroke and or coronary heart diseases. Currently, the hypertension rate in Kansas is estimated to be 31.3%. This is alarming as the state is ranked as the 27th in relation to this obesity-related health concern (Trust for America's Health and Robert Wood Johnson Foundation, 2014).

Older adults with lower education levels are not able to monitor their blood pressure, therefore, medical practitioners should focus on aiding such individuals. These medical practitioners can focus on educating the community on the risks, causes, and treatment plans for the condition. In regard to the setting, resources that are available in this community that can aid in meeting the requirements of the target population include education, group counseling, and community empowerment fairs at senior homes. Dire areas of education in the prevention and management of this condition include increasing the nutritional standards and engaging in regular exercise programs. Programs such as farm-to-school programs and health assessment and screening ought to be implanted in this area (McDonald, Hertz, Unger, & Lustik, 2009).
