System Evaluation and Quality Improvement Introduction Adequate evaluation of healthcare information systems and processes is vital to necessitate the systems to meet the requirements and information processing needs of users and healthcare organization adequately. The purpose of this paper is to elucidate on the evaluation and quality approaches that exist, it includes discussion of both informative and summative evaluation. In addition, the paper shall consider both qualitative and quantitative approaches to evaluation. This includes coverage of survey methods, observation, interviews and focus groups. Incidents are usually unplanned occurrences that have consequences in losses such as fatalities or injuries, causing assets damage, equipments, surroundings, business failure or the image of a company to its customers (David, House m.d, 15th may 2007). A near miss is an incidence that could have resulted in the mentioned losses; however the events that results to the incident stops in time before the occurrence of the event. There many ways that can be used to describe an event depending on the investigations and cause of the incident. House –Episode 23 (Season Three): "The Jerk." The Episode is characterized by a character known as Nate; he is an outgoing but annoying young boy who is a renowned chess player. He was competing in a chess tournament speed. Luckily he manages to win the tournament easily, but sadly, he is overpowered by emotions and began physically assaulting his opponent until the opponent becomes unconscious. He starts complaining of a severe headache, and he is rushed to the hospital. Unfortunately, his condition worsens, and he is admitted to hospital (David, House m.d 15th may 2007). After admission to the hospital, several tests are carried out to diagnose the problem that might have been affecting him. The initial differential diagnosis done on him includes test of any possibility of parasites, but the test proves to be contrary, brain tumor where magnetic resonance imaging is conducted, and the test was also negative. In addition, toxins and drugs tests were performed, and the toxin screen was negative. Another physician Foreman suggested that the patient be tested of any likelihood of adrenal gland tumor (David, House m.d 15th may 2007). However, House was suspecting that Nate was suffering from cluster headaches. According to House, the patient was to be put on blood thinners and transcranial magnetic stimulation. The quality of medical service offered to Nate was below the expected standards. Therefore, the quality of the services offered to him was low. If the diagnosis done to him respected the requirement of quality, the right diagnosis would have been obtained. Sadly, the therapy accorded to him proves to be futile, and no changes are seen in the Nate’s condition. His situation continues to worsen and raises more and more complaints of headaches. Nate situation went out of control, and he started complaining of a shoulder, face and stomach pains (David, House m.d 15th may 2007). However, the latest pains would be as a result of the fights he had been engaged. But, it remains unclear on the correlation between fights and stomach ache. The correlation is thus said to be under qualified. House maintains his assertions that Nate was suffering from cluster headaches. Without considering the quality of test done onto him.Cameron came with a different suggestion that was a condition known as hemochromatosis this was with an aim of establishing a good quality medicare. This is a disease associated with iron storage deficiency. However, House disclaims the idea by Cameron by saying that the state would not explain the personality changes. Additionally, Chase suggests a condition of Hypothyroidism. But House was first to reject the suggestion by seeking his support from the patient lack metabolic slowing as a symptom most prevalent in individuals with cases of low thyroid levels. House suggests that the best treatment was to offer therapy to Nate, which includes psychedelic mushrooms because the mushrooms contain a component known as psilocybin that is very powerful in preventing cluster headaches. Interestingly, the mushroom helps to reduce his cluster headache but the mushrooms seem to stimulate his testosterone and he ends up becoming highly sexually activated, and his sexual feelings accelerate. House later flashes his equipment to Cameron; she becomes elated after noticing he is a swell suffering from hypogonadism that is a condition characterized by abnormally small underperforming testicles. The primary interest now is hypothalamic lesion or a craniopharyngioma. They have eliminated all the other possibilities and zeroed in with only this condition. From the cascade of events, it is true that they have initiated good quality tests. As a result, House instructed to be provided with biopsy of Nate’s pituitary gland. The differential diagnosis of liver failure is complicated and takes time to be accomplished. The diseases examined from him include Wilson’s disease that is a disease that is characterized by the lack of sufficient copper metabolism by the relevant enzymes (David, House m.d 15th may 2007). Also, tests were done on any possibility of cancer more specifically examining for the presence of any inflammatory disease of the bile ducts. In addition, test on alcohol abuse and ornithine transcarbamylase (OTC) deficiency that is a genetic condition that tampers with an individual’s ability to remove excess nitrogen from the body were also tested. Since all the differential tests conducted on him none had provided a clue to the Nate’s condition, they decided to carry out an elimination differential diagnosis. Some of the tests performed to rule out different conditions include “Hamburgers Tests” but the results were also negative. In addition, he ordered Young Guns to stop any food from reaching Nate to rule out Diabetes Steatosis. Diabetes steatosis is a condition in which is a condition characterized by fats accumulation in the liver as a result of diabetes, the fats are abnormal. Nate turns to be violent, I some instance he is requested to give urine samples, and he ends up urinating on the floor (David, House m.d 15th may 2007). He is found to have samples of blood in his urine which an evidence he might be suffering from kidney failure that is already developed. There was no any condition successfully diagnosed at this point. Physicians conducted differential diagnosis on HIV/AIDS and a condition referred to as hepatic fibrosis and another genetic disease characterized by severe sickness when the patients miss their meals. This is a common test done to any patient who reports any cases of repeated illnesses therefore it is not a quality diagnosis. The last diagnosis to be conducted on him was Von Gierke Disease (David, House m.d 15th may 2007). However, despite all the efforts done to establish what was ailing him, all the tests were negative. House then decided to test Nate by supposedly inducing anger in him; he does this by engaging him in a game of chess while being offensive to him through taunting him. The test does not meet the set standards of quality. After this practice, House loses the game but Nate develops a seizure. To start the therapy, he exposes Nate to immunosuppressant’s and starts searching for a door leading to the bone marrow. But Foreman takes caution and performs a biopsy test for amyloidosis, and the tests were unlikely negative (David, House m.d 15th may 2007). However, House believes in himself and proposes that it might be Foreman’s laxity and made errors in the biopsy and sent him again to repeat the biopsy test. Luckily, a broad discussion with Chase makes them come up with the actual diagnosis that is hemochromatosis (David, House m.d 15th may 2007). The uncouth personality and bad behaviors by Nate were not as a result of any condition, but he was just a Jerk. This is the best and the highest quality test. Through the discovery of the right diagnosis it shows that after all the hustles they have established the right quality diagnosis. Constellation Diagram Recommendations in terms of importance It is expected that, in modern times medicine ought to be useful and more efficient than it was in the past. However, medicine is still a field that has not developed as many would expect. In the film, there was a great deal of moving from one diagnosis to the next without precise sequences of events. The first recommendation touches on the general health of the patient.The entire cluster headache case was mishandled both professionally and technically. Therefore , when handling such cases , there is a need to approach them in the most professsional and technical way possible. The second recommendation is based by the fact that the physicians failed to understand that the symptoms that he had portrayed did not necessitate all the diagnosis that was done on him (David, House m.d 15th may 2007). There is hence a need to be authentic when carrying out various tests as evidenced by the time wasted to perform all the futile diagnosis. If more time were lost before Nate was diagnosed with that condition, it would have been worse since there was a possibility of death occurring. Hence, the last recommendation in would propose is that there is a need for the healthcare providers to ensure that no time is lost which can cause life loss.
