The facility has specialized in educating the patients on self-care techniques that can be effective when they are back in their nursing homes. It also provides the patients with long-term-care that is within the standards of nursing and also providing them activities that aid their comfort, recovery, and stay in the facility. (CareOne, 2020). Notably, as with all healthcare institutions, the aim of CareOne at Randolph is to provide quality healthcare. Thus, in the past years, it has been the leading patient care facility in terms of quality services in Massachusetts. It's crucial to note that the facility is large and has 168 beds. Improvement Opportunity

Nursing documentation is essential in enhancing quality service delivery and the success of patient care facilities. Keeping records of the patients and the equipment bought by the facilities together with all its assets is essential for the organization's growth. It's crucial to note that service care delivery should be factual, comprehensive, and current and provide consistent information about the care provider and the reviews and responses of the patients before and after treatment. (Obioma, 2017). According to Collins et al. (2013), there's a relationship between nursing documentation and patient mortality. Nurses are prone to alter their patient monitoring behavior as their condition deteriorates by showcasing minimal records. The insufficient information at this moment provided would be, in essence, act as a variable in increasing patient mortality. In CareOne hospitals, registering over 20000 patients annually, keeping clear records of the patients would be severe.

It's crucial to note that the essence of proper documentation by nurses is to provide assessment, identify clinical needs and problems of the patient, the forecast of the plans of care that would be effective in giving a patient suitable environment, communication with another healthcare professional regarding the conditions of the patient. (American Nurses Association, 2010). Proper documentation also serves to provide patient outcomes and responses, records of patient medication, patient clinical parameters, and even order the implementation of new strategies in enhancing the quality service provision. The other crucial aspects of documentation are aiding legal fact-finding, offering to credential, verifying reimbursement services, research, and the facility's regulation. (American Nurses Association, 2010). Therefore, it's crucial to improve the documentation systems in nursing in CareOne at Randolph, Massachusetts. Given the high number of patients administered in the facility, it's difficult not to omit certain documentation aspects. Conclusively, the documentation system should be improved to aid the general quality service delivery.

Purpose

Given the challenge of administering several patients annually, CareOne at Randolph requires a new technique that would enhance the proper use of documentation. The essence of his research paper is to provide appropriate documentation and how it can be useful in retrieving information that can reduce mortality and improve the service delivery of the healthcare setting. Proper documentation is the process of making and keeping records on the progress of the patients and also aid the decision-making mechanism in enhancing the safety of the patients. (Mutshatsi, 2018). Record keeping is not correctly conducted in a variety of health facilities, and thus the patient safety is not adequately enhanced. The essence of every patient care facility, especially those dealing with chronic illness patient care services like the CareOne at Randolph, Massachusetts, reduces the mortality and enables the patients to acquire better services. The challenges of improper documentation lower patient safety through omission, misplaced documentation, and even mixed messages. (Butler, 2015). It's thus essential for CareOne at Randolph Massachusetts to incorporate improvement in their recording and documentation procedures towards the enhancement of offering quality services of health care.

Proposal Initiative

My proposal for CareOne at Randolph is to develop an effective strategy that would enhance the proper use of documentation. Several techniques can be employed in literature by the nurses. These techniques include the weekly services for the certified nursing assistants, where they can record each patient's details and provide an overview of their progress every week. They can then pinpoint missing documentations and thus make an addendum within 15 days of the missing data. Notably, the best strategy that would be effective in the proper use of documentation would be the implementation of Electronic Health Records with digitized patient records by the nurses. The EHR can work well with software packages that are user friendly and can be employed in accessing documentation, evaluation of outcomes, and seamless communication between the members of the organization and also provided for revision and improvement of the documentation process. Notably, the software can also offer databases updated twice a month, capable of proving all the healthcare records.

Leadership

In implementing this proposal, it's crucial for the leadership of CareOne at Randolph to incorporate all the stakeholders in the organization. Notably, CareOne is a family-owned organization. However, it has its leadership structures in performing managerial roles. The administration should put into consideration this proposal because it will serve to improve the proper ruse of documentation and also create a system of easy access and update of records inclusive of the missing files. The current leadership style at CareOne Massachusetts is democratic. His implies that the managerial staff can incorporate all the stakeholders involved, namely, the subordinate and professional staff, the family that owns the organization, and the board of directors. The organization's leadership also needs to develop evaluation and monitoring strategies to ascertain the progress of the implementation exercise. Conclusively, the leaders will be receptive to the change because it will serve to improve the quality of their services.

Conclusion

The essence of every healthcare setting is to provide quality services while keeping its doors open. CareOne at Randolph, for instance, is one of the leading chronic illnesses patient caring facilities in the United States. It’s known to deal with a variety of chronic illnesses patient, caring techniques. The facility has over 168 beds with an estimated annual admission of 20000 patients. Notably, it would be difficult to institute proper use of documentation given a large number of patients, and this could lead to a decrease in the safety of the patients and, in turn, increased mortality. Therefore, there is a need to improve the use of the facility's documentation procedures mainly through EHR. Also, software databases would be essential in enhancing the proper use of documentation.
