In the healthcare industry, effective discharge of patients is decided by doctors after analyzing the condition of patients. However, the planning can be completed by a nurse, social worker, case manager, or any other healthcare professional 15. The basic discharge plan includes evaluation of the patient by a qualified healthcare professional, discussion with the patient and the concerned representative, planning for transferring patients to another care unit, determining the overall training requirement for the caregiver, referrals to home care agency, and arranging to follow ups and appointments for further tests. Michels stated that effective discharge decreases the chances of patients being readmitted along with saving their time, money, and energy in a hassle-free manner. A number of researchers found that in the past patients were discharged quicker and sicker making it all the more critical in terms of better discharge 11.

It is often believed that effective discharge of patients is dependent on a number of strategies that need to be assessed and analyzed by the hospital management in an efficient manner. At the same time, effective discharge is dependent on the cooperation and coordination between the healthcare professionals and medical staff. Though strategies and planning for patients’ discharge are generally similar for the majority of hospitals discharge policies for different patients belonging to different age categories may be different. In this regard, the major purpose of this research work is to assess and investigate factors that affect the effective discharge of older people from hospitals. There can be a number of internal and external factors regarding this and the research will be based on analyzing those factors in a critical manner.
